Alongside our Physicians, we have nurses, medical assistants, and scribes to ensure that our patients feel connected to their experience with Neighborhood and that they feel heard as a patient and their needs.
This particular role is focused exclusively on our PACE Participants who receive tailored medical care from the finest professionals who specialize in aging and chronic conditions. Our full team consists of therapists, dieticians, social workers, and home care nurses to create a customized care plan for each NHC PACE participant to ensure optimal health, safety and well-being.
ESSENTIAL DUTIES/RESPONSIBILITIES
- Review patient file/record, including allergies, problems, medications and immunization status.
- Elicit and record information about patient’s medical history.
- Examine patients for symptoms or physical information.
- Order and execute various test, analyses and diagnostic images to provide information on patient’s condition.
- Analyze reports and findings of tests and examination and diagnoses condition of patient.
- Determine and prescribe medication, dosage and schedule given the patient’s condition and allergies.
- Discuss potential side effects to medication or immunization with patient.
- Promote health by advising patients about diet, hygiene and methods for prevention of disease.
- Supervise Advanced Practice Providers (Nurse Practitioners/Physician Assistants)
- Perform surgical procedures and commensurate with surgical competency, when appropriate for specialty and training.
- Refer patients to medical specialist for consultant services when necessary for patient’s well-being.
- Follow up with patient regarding progress in high risk or emergency cases.
- Conduct physical examinations to provide information needed for admission to school, consideration for jobs or eligibility for insurance coverage.
HIPAA
- Maintain privacy of all patient, employee and volunteer information and access such information only on a need to know basis for business purposes.
- Comply with all regulations regarding corporate integrity and security obligations. Report all behaviors and/or activities that are unethical, fraudulent, or unlawful.
Requirements
Education/Licenses/Certifications:
- Current CA Medical License (MD/DO)
- DEA License
- CPR Certification
- Obtain and maintain board certification
- CPR/BLS certification
Experience:
- Experience treating adult geriatric population
- PACE experience preferred.
Salary: $250,000 to $315,000 DOE
